Welcome to Ridgecrest Village. Here is an
outline of what to expect when you visit, and how to apply for admission. We'll
be happy to talk with you about your retirement plans. Call our office at 563-391-3430 or visit us anytime 9 a.m.-3:30 p.m. weekdays. Evenings and weekends by appointment.

You will be treated to a personal tour of
our community. Your family is also welcome, if you like. This will give you and
your loved ones an opportunity to learn about Life Care, meet our residents and
staff and get a good taste of the atmosphere here at Ridgecrest. Feel free to
ask questions of residents and staff at any time during your visit.

A Ridgecrest admissions staff member will
help you select your private cottage or apartment and go over the required
forms:
Ridgecrest Application/Confidential
Financial Statement
Personal Health History
Report of Physical Examination (to be
completed by your physician and submitted to Ridgecrest Village)
A fully refundable 10% deposit is required
to formalize your cottage or apartment reservation.

Your application will be reviewed by a
qualified committee which includes our Board of Directors. The committee
determines Ridgecrest's ability to serve you appropriately based, in part, on
review of Personal Health History and Physical Examination forms. The applicant
should be in reasonably good health and be able to live independently. The
applicant may utilize our Home Health Care Program. You will be notified of
approval by a Ridgecrest admissions staff member. Neither race, color, sex, nor
national origin has any bearing upon your acceptance for admission to Ridgecrest
Village.

You and Ridgecrest make plans for your move.
Your reserved apartment or cottage will not be shown to any other applicant
during this time.

The balance of the entrance fee is due on or
before the date you move in.
